Important Information

I understand that this form is to request a bike for my child or to refer another family's child to receive a bike. Bikes are for boys and girls ages 7-12 years old. Requests for children outside of those age ranges can not be filled. Please submit a separate form for each child. Child and parent must be present at Propel Church on December 22nd for the 10:30am Worship Experience. (THIS IS A SUPPRISE AND I WILL NOT INFORM THE CHILD)
